Glycated albumin guidance in early diabetes treatment shows limited impact on standard HbA1c target but benefits metabolic measures in Chinese trial

A Chinese multicentre trial reveals that using glycated albumin to guide early treatment of newly diagnosed type 2 diabetes does not improve HbA1c target achievement but enhances metabolic control and could influence treatment strategies.

A multicentre randomised trial in China has found that guiding early treatment of newly diagnosed type 2 diabetes with glycated albumin did not improve the share of patients reaching the standard HbA1c target of below 7% over 12 weeks, but it did appear to help with tighter glycaemic control and several other metabolic measures. The study enrolled 200 adults across seven hospitals and compared a strategy that intensified treatment when glycated albumin stayed above 16% at four weeks with standard care, in which doctors adjusted therapy without seeing the glycated albumin results.

By the end of the study, both groups had almost identical rates of HbA1c below 7%, at about 64%. But the glycated albumin-guided group did better on a stricter HbA1c target of below 6.5% in the per-protocol analysis, and it also achieved a larger fall in HbA1c overall. The same group showed greater improvement in beta-cell function, lower insulin resistance, and larger reductions in body weight, body mass index and waist circumference. Rates of hypoglycaemia remained low in both arms.

The findings add to a growing body of evidence that glycated albumin is a useful short-term marker of glucose control. Reviews and clinical studies have described it as a measure that reflects recent blood sugar changes more quickly than HbA1c, making it potentially helpful when treatment is being adjusted rapidly or when HbA1c is less reliable, such as in anaemia or haemoglobin disorders. Earlier research has also linked glycated albumin closely with fasting and post-meal glucose levels and suggested it may be more sensitive to short-term changes during treatment.

The study authors said the results do not establish glycated albumin guidance as superior for reaching the common HbA1c goal, but they argue it may still have practical value in pushing earlier combination therapy and in supporting more ambitious treatment targets. They also cautioned that the trial was short, limited to newly diagnosed patients in China, and included relatively few people using injectable therapies, so broader research will be needed before glycated albumin can be firmly established as a routine treatment guide.
