Scientists in Shanghai have created an oral probiotic capable of detecting and responding to blood sugar fluctuations, signalling a new move towards adaptive, personalised diabetes treatment.
Researchers in Shanghai have developed an oral probiotic designed to detect changes in blood sugar and react in real time, in what could become a new approach to diabetes care. The work, led by scientists at East China Normal University, uses synthetic biology to turn a familiar probiotic strain into what they describe as a glucose-sensitive system that can both sense and respond to metabolic shifts.
According to the study, published in Nature on 12 August, the modified microbe carries a glucose-recognition and functional-response system known as GIFT. When taken by mouth, it is designed to monitor blood glucose fluctuations and release hormones that help lower sugar levels only when needed. That would mark a shift away from fixed-dose treatment towards a more adaptive model that follows the body’s changing needs.
The idea builds on a wider body of research suggesting that probiotics and related synbiotic therapies, which combine probiotics with prebiotics, may help manage type 2 diabetes by influencing gut bacteria, improving insulin sensitivity and reducing inflammation. Separate studies published in Nature journals have found that unmodified probiotics can affect glucose metabolism in people with diabetes, while bioengineered strains in animal models have been shown to reshape the gut microbiota and improve glucose control.
The attraction of the Shanghai team’s approach is its promise of a biological feedback loop: a living treatment that could operate like a miniature control system inside the body. That vision is still preliminary, and the researchers say further work is needed to establish safety and effectiveness, but the concept reflects a broader push in so-called smart medicine towards therapies that can detect what the body needs and respond automatically. With diabetes now affecting more than 530 million adults worldwide, according to the International Diabetes Federation, and China accounting for about 120 million cases, the search for more personalised treatment options remains urgent.
