D&D Pharmatech's liver drug may improve organ health before weight loss, challenging current treatment strategies

D&D Pharmatech’s experimental drug zabopegdutide shows promising signs of improving liver health independently of weight loss, potentially revolutionising treatment for metabolic liver diseases.

D&D Pharmatech says its experimental liver drug zabopegdutide may be doing more than helping patients slim down. According to the South Korean biotech, 12-week data from a Phase 2 study published in The Lancet Gastroenterology & Hepatology showed that the treatment improved signs of liver health well before most participants had lost substantial weight, raising the possibility of a direct effect on the organ itself.

That matters because metabolic dysfunction-associated steatotic liver disease, and its more severe form metabolic dysfunction-associated steatohepatitis, or MASH, are widely seen as diseases driven by obesity and insulin resistance. Weight loss has therefore been one of the main therapeutic goals, but the latest findings suggest that liver benefit may begin earlier than previously assumed, and may not depend entirely on the scales.

In the 12-week analysis, nearly three-quarters of patients on zabopegdutide reduced liver fat by more than half, while almost half reached levels regarded as normal. Investigators also reported improvements in measures associated with liver stiffness and fibrosis. Even among patients who had lost less than 5% of body weight by week 12, the drug still appeared to improve liver fat and stiffness, suggesting a possible dual mechanism: direct liver activity alongside the benefits of weight reduction.

The new paper builds on D&D Pharmatech’s earlier 48-week biopsy read-out, which the company said showed statistically significant improvements in both fibrosis and MASH resolution. In a statement quoted by the company, chief executive Seulki Lee said the longer-term results were “foreshadowed by rapid and substantial improvements in liver health within the first 12 weeks of treatment”, adding that many of those gains came before meaningful weight loss. The once-weekly therapy was also described as generally well tolerated, with a short two-week dose-escalation period before patients reached the target dose.

The timing of the publication is notable because the MASH field has become increasingly crowded, with developers now judged not only on whether a drug works, but on how it works and which patients it may suit best. If further studies confirm that zabopegdutide can improve liver disease independently of major weight loss, it could stand out in a market where many treatments still depend on broad metabolic effects. For longevity researchers, the implication is broader still: future therapies may need to protect organs directly, rather than relying on weight loss as a proxy for better health.
