Patient safety probe exposes critical gaps in insulin support for people with learning disabilities

A new investigation highlights persistent inequalities and safety risks for people with learning disabilities managing insulin, amid calls for improved community support and evidence-based practices.

A new patient safety investigation has warned of “inequitable” support for people with a learning disability who self-administer insulin, saying gaps in care have left some patients at risk of avoidable harm. The Health Services Safety Investigations Body said its findings showed that too many community services are still not built around the needs of people with disabilities, despite the fact that many can manage their diabetes safely when they receive the right support.

The report, published on 19 May, examined the experience of a woman identified as Ms H, who lives in residential care and has type 2 diabetes. With specialist help, she had learned to inject her own insulin, gaining more independence and flexibility in daily life. But investigators said a change in insulin pen device led to a period in which she did not realise she was not getting the medicine correctly, underlining how device design, supervision and communication can all affect safety.

According to HSSIB, people with a learning disability have been harmed when support for insulin self-administration was inadequate, while others were not properly enabled to manage their own treatment when they could have done so. The body also pointed to limited access to learning disability specialists, weak evidence on what support works best, and possible under-reporting of insulin-related incidents in community settings. It has asked the National Institute for Health and Care Research to address the evidence gap, while senior safety investigator Nick Woodier said the findings showed recurring problems across healthcare, including poor access to support, gaps in staff knowledge, communication failures and the uneven use of reasonable adjustments.
