GLP-1 medicines linked to reduced fracture risk in type 2 diabetes patients

A large observational study suggests that GLP-1 receptor agonists may offer protective benefits against fragility fractures in older adults with type 2 diabetes, raising hopes for broader skeletal health effects beyond glucose regulation.

A large observational study has added to evidence that GLP-1 medicines may do more than help manage blood sugar and weight. Researchers analysing more than 133,000 adults with type 2 diabetes found that those who began a GLP-1 receptor agonist had a 21% lower risk of fragility fractures over three years than patients starting a different diabetes drug class, according to findings published in JAMA Network Open.

The study used electronic health records from the TriNetX Research Network and compared people aged 50 to 90 who started either a GLP-1 drug or a DPP-4 inhibitor, a separate type of diabetes medicine. After matching patients on factors that could influence fracture risk, the two groups each included 66,803 people. The strongest reductions were seen in fractures of the spine, hip and femur, and the association remained even after researchers adjusted for changes in body mass index and blood sugar, suggesting the benefit may not be explained by weight loss alone.

That said, the result does not prove the medicines directly strengthen bone. Because the analysis was retrospective, it can show an association but not causation. The study’s authors said further prospective research is needed to determine whether GLP-1 medicines truly protect bone and to understand any longer-term effects on skeletal health. A separate study in The Journal of Clinical Endocrinology & Metabolism reported a similar fracture reduction in older adults with type 2 diabetes, while a 2019 meta-analysis and a more recent review in Bone Research suggested GLP-1 drugs may have a neutral or beneficial effect on bone health.

The findings come as GLP-1 medicines, already widely used for diabetes and weight loss, continue to draw attention for effects beyond glucose control. That matters because rapid weight loss has raised concerns about whether bones could become more vulnerable. For now, the new research offers reassurance for people with type 2 diabetes, but not a final answer.
