New research and medical approaches are offering hope to those suffering from persistent diabetic neuropathy, with spinal cord stimulation emerging as a promising option for pain relief beyond traditional drugs.
For many people with diabetes, neuropathy is not an abstract complication but a daily intrusion: burning feet that make sleep difficult, stabbing jolts that travel up the legs, or numbness that makes walking feel unsafe. Kayvan Haddadan, a pain physician, argues that the common belief that nothing more can be done is wrong. Diabetic neuropathy is the most common form of peripheral nerve damage in people with diabetes, and Mayo Clinic says tighter blood-sugar control remains a central part of care, alongside treatment for pain and function.
The condition develops when high glucose gradually harms the long peripheral nerves first, especially those reaching the feet and hands. That damage can trigger not only numbness and tingling, but also weakness, balance problems and problems affecting blood pressure, digestion, sweating and sexual function. As Haddadan notes, pain can also be amplified by the spinal cord and brain, which helps explain why symptoms may be severe even when the nerve injury itself seems limited.
Standard treatment usually begins with a broad approach: better glucose control, attention to blood pressure and lipids, foot care, exercise and the search for reversible causes such as vitamin deficiency or thyroid disease. WebMD and Mayo Clinic both describe commonly used medicines such as gabapentin, pregabalin, duloxetine, venlafaxine and low-dose tricyclic antidepressants, while topical options such as capsaicin and lidocaine can help some patients. Healthline has also reported on red light therapy as a possible non-invasive option, although it says the evidence is still limited and more research is needed.
For patients whose pain does not respond to drugs alone, the treatment landscape has broadened. A systematic review indexed in PubMed found growing evidence for neuromodulation approaches, including spinal cord stimulation, in painful diabetic neuropathy. Haddadan says such procedures can provide substantial relief for selected patients, particularly when other treatments have failed. He describes the goal not as perfect cure, but as better sleep, easier movement and a reduction in the burden of daily pain.
