New research reveals brain pathway may enhance metformin’s blood sugar control

Scientists discover that metformin’s glucose-lowering effects may involve direct action in the brain, opening prospects for more targeted diabetes treatments.

Scientists say a common diabetes medicine may do more than work through the liver and gut. A Baylor College of Medicine study suggests metformin also acts in the brain, revealing a pathway that could help explain how the drug lowers blood sugar and open the door to more targeted treatments.

According to Science Daily, the research centres on a region of the brain called the ventromedial hypothalamus, where metformin appears to suppress a protein known as Rap1. That change activates SF1 neurons, which the researchers say are directly involved in the drug’s glucose-lowering effect. In comments reported by Science Daily, Makoto Fukuda said the findings show that the brain responds to much lower levels of metformin than the liver or intestines.

The team tested the theory in genetically engineered diabetic mice and found that metformin delivered directly into the brain lowered blood sugar even at doses far below those taken by mouth. The study’s authors say this suggests the brain is not just a passive bystander in metformin’s action, but part of the medicine’s core mechanism.

Metformin is one of the world’s most widely prescribed drugs and is used mainly for type 2 diabetes, though it is also given in some cases of gestational diabetes and prediabetes. It is generally favoured because it does not usually cause weight gain, but the NHS notes it can still cause side effects. The new findings, if confirmed in further research, could help scientists design treatments that work more precisely by targeting the same brain pathway.
