Meta-analysis links artificial sweeteners to increased type 2 diabetes risk amid rising health concerns

A comprehensive analysis of over 720,000 adults reveals a potential connection between heavy artificial sweetener use and higher chances of developing type 2 diabetes, prompting renewed debate over sugar substitute safety.

A new meta-analysis drawing on data from more than 721,000 adults suggests that heavier use of artificial sweeteners is linked with a higher risk of type 2 diabetes, adding to debate over whether these sugar substitutes are truly metabolically neutral. The review, published in Frontiers in Nutrition and summarised by News-Medical, pooled six long-term cohort studies with an average follow-up of just over 10 years.

The analysis covered participants in the United States, Japan and Europe, including major cohorts such as NutriNet-Santé, EPIC-InterAct, MESA, the Japanese Public Health Center-based study, the Health Professionals Follow-up Study, and the Nurses’ Health Study. Across the studies, 38,445 cases of type 2 diabetes were recorded. Researchers found that people with the highest intake of artificial sweeteners had a 31% higher risk of developing the disease than those with low or no intake, although the evidence was rated low certainty because the studies were observational and differed substantially in design and exposure measures.

Earlier research points in a similar direction. A 2023 study in Nature Food reported that higher consumption of aspartame, acesulfame-K, sucralose and total artificial sweeteners in the NutriNet-Santé cohort was associated with increased diabetes risk, even after excluding early diagnoses. A separate study in Diabetes Care also found a positive association between artificial sweetener intake and type 2 diabetes in the same French cohort. Taken together, the findings do not prove causation, but they do suggest that the long-term effects of sweeteners may deserve closer scrutiny than many consumers assume.

The latest review also explored possible explanations. The authors said artificial sweeteners have been tied to changes in the gut microbiome, which may influence insulin response, intestinal barrier function and inflammation. They also noted possible effects on appetite and energy-regulation pathways. Still, the researchers cautioned that some people may choose artificial sweeteners because they already have excess weight or are at higher metabolic risk, which makes it difficult to separate cause from effect. For now, the evidence suggests caution rather than certainty.
