Midlife exercise and blood sugar management may delay cognitive decline in later years

A recent study suggests that focusing on physical activity and blood sugar regulation in midlife can significantly slow cognitive decline among adults, with implications for dementia prevention.

A midlife focus on exercise and blood sugar control may help slow cognitive decline later in life, according to a study reported by Sky News Arabia and published in Alzheimer’s & Dementia. The research followed 402 adults who did not show signs of cognitive impairment at the start and assessed their heart-health profile using seven measures, including diet, smoking, blood pressure, cholesterol, blood sugar and body mass index.

After years of follow-up, two factors stood out most clearly: physical activity and blood glucose. The report said greater activity was linked with slower cognitive decline among participants of Mexican American background, while healthier blood sugar levels were tied to slower decline among white participants. The findings add to a growing body of evidence that cardiovascular health in midlife can influence how well the brain ages.

The broader scientific literature points in the same direction. Earlier research published in Alzheimer’s & Dementia found that physical activity in midlife was associated with better later-life cognition and a lower risk of dementia, while a long-running Whitehall II study in the BMJ linked higher activity levels with slower cognitive decline. Other studies have also connected better cardiorespiratory fitness with less brain atrophy and slower decline in people at risk of Alzheimer’s disease.

Specialists quoted in the report said the biology is plausible: glucose provides energy for the brain, while exercise improves blood flow and oxygen delivery. Gail Livingstone of University College London said very low blood sugar can harm nerve cells, while high levels are associated with damage to blood vessels and reduced circulation to the brain. She advised adults to aim for 150 minutes of moderate exercise a week, or 75 minutes of vigorous activity, and to check blood sugar rather than relying on symptoms alone. Experts also stressed that brain protection depends on more than exercise and glucose, with blood pressure, cholesterol, diet, smoking, obesity and sleep apnoea all playing a role.
