An international study predicts a 29% increase in people living with type 1 diabetes by 2049, driven more by better care and environmental factors than obesity, raising new concerns over its changing epidemiology.
Type 1 diabetes is set to become more common worldwide over the next quarter-century, but researchers say the rise is being driven less by obesity than by better survival and a growing number of new diagnoses. An international team presenting findings at the European Association for the Study of Diabetes estimated that the number of people living with the condition will climb by about 29% by 2049, from roughly 9.4 million today to 12.1 million. They also forecast a 14% increase in new cases over the same period.
The distinction matters because type 1 and type 2 diabetes are often confused. According to the Cleveland Clinic and other medical references, type 1 is an autoimmune disease in which the immune system destroys the pancreas’s insulin-producing beta cells, leaving patients dependent on insulin for survival. Type 2 diabetes, by contrast, is usually linked to insulin resistance, when the body cannot use insulin effectively, and is more closely associated with excess body weight and lifestyle factors.
Researchers say the projected rise in prevalence reflects progress in care. Modern insulin formulations, continuous glucose monitors and insulin pumps have helped people with type 1 diabetes live far longer than in the past, when the diagnosis carried a much higher risk of early death. That means more people are living with the disease for decades, adding to the global total even if the underlying biology has not changed.
The expected increase in new cases is harder to explain. Scientists say it cannot be reduced to genetics alone, because population genetics do not shift quickly enough to account for the trend. Instead, they point to a mix of inherited susceptibility and environmental triggers, including viral infections, changes in gut bacteria, infant feeding patterns, air pollution and exposure to some chemicals. The exact cause remains uncertain, but experts say the evidence suggests multiple factors acting together, especially in early childhood.
