Metformin’s potential to slow biological ageing gains new scientific backing

Recent research suggests that metformin, a widely used diabetes medication, may influence cellular pathways related to ageing, sparking renewed interest in its wider health benefits beyond blood sugar control.

Metformin, a mainstay treatment for type 2 diabetes for decades, is drawing fresh attention for reasons that go beyond blood sugar control. A review published on 10 August in the journal Aging suggests the drug may also affect biological pathways linked to cellular ageing, adding to a growing body of research on its possible wider health effects.

The drug is widely used as a first-line therapy for type 2 diabetes and is listed by the World Health Organisation as an essential medicine. Type 2 diabetes is a long-term condition marked by insulin resistance and elevated blood glucose, and it is often associated with obesity, older age and the need for long-term lifestyle changes alongside medication.

Researchers behind the review say metformin appears to work through several mechanisms at once. One of the key targets is AMPK, an enzyme involved in cellular energy balance. By influencing this pathway, the drug may help lower glucose levels while also reducing oxidative stress, improving insulin sensitivity and promoting autophagy, the process by which cells clear out damaged components. The review also points to effects on mTOR, a protein involved in growth and metabolism, which has been linked in experimental models to longer lifespan when suppressed.

Earlier reviews indexed by the US National Library of Medicine describe metformin as acting on multiple recognised “hallmarks of ageing”, including DNA damage, inflammation, mitochondrial dysfunction, cellular senescence and changes in gene regulation. Some animal studies have also linked the drug to longer life and better physical performance, while other research has examined its effects on the insulin and insulin-like growth factor pathways.

Human evidence remains far less certain. One 2022 study involving 32 patients with type 2 diabetes found signs of slower biological ageing among metformin users, with an estimated difference of 2.7 to 3.4 years, but the authors cautioned that such findings do not prove cause and effect. The review published in Aging says the same limitation applies more broadly: many studies look at separate outcomes such as cardiovascular disease, cancer or mortality, making it difficult to draw one clear conclusion about ageing itself.

The authors also highlight possible links between metformin, the gut microbiome and inflammation. Experimental work suggests the drug may alter intestinal bacteria in ways that improve metabolic markers, and in animal studies those changes have been associated with slower tumour progression in high-fat diet settings. Still, the review concludes that more work is needed, particularly in people without diabetes. One major effort to answer that question is the planned TAME trial, which is expected to enrol about 3,000 participants across 14 research centres in the United States and follow them for several years.
