Walkable neighbourhoods linked to lower diabetes risk after pregnancy in affluent areas

Research from Columbia University reveals that women living in more walkable New York City neighbourhoods are less likely to develop type 2 diabetes following gestational diabetes, especially in affluent areas, highlighting the importance of neighbourhood design for long-term health.

Researchers at Columbia University’s Mailman School of Public Health have found that women who lived in more walkable New York City neighbourhoods were less likely to develop type 2 diabetes after gestational diabetes mellitus, according to a study published in Diabetes Care. The work adds to evidence that the design of the places people live in can shape long-term health after pregnancy, not just during it.

The study followed 12,403 women with gestational diabetes identified through birth records, hospital discharge data and laboratory A1C results, then tracked outcomes through 2021. Over 12 years, 21% developed type 2 diabetes. After adjusting for age, race and ethnicity, education, nativity, parity and neighbourhood deprivation, women in areas with medium walkability had a 15% lower rate of type 2 diabetes than those in less walkable neighbourhoods, the journal reported.

The pattern was not uniform. The benefit linked to highly walkable neighbourhoods appeared only among women living in the most affluent areas, suggesting that walkability and income context interact. Bohao Wu, the study’s first author, said the research helps fill a gap in understanding how individual and neighbourhood factors jointly shape diabetes risk after gestational diabetes. Senior author Teresa Janevic said planning for walkability may matter most when it is paired with efforts to reduce economic disadvantage.

That broader view fits Columbia’s earlier work on neighbourhood design and metabolic health. The university’s Built Environment and Health Research Group has reported links between walkability and physical activity, body-mass index, gestational weight gain and glycaemic control, while a separate Columbia study found higher walkability was associated with lower gestational diabetes risk. Taken together, the findings point to a simple but important idea: streets that are easier to walk may support better health, but the gains are likely to be strongest when residents also have safe, clean and economically stable surroundings.
