According to Cambridge researchers, the work suggests sweeteners may not be as metabolically neutral as many consumers assume.

The team tested 39 commercially used sweeteners against 25 gut bacterial species in laboratory conditions and found that about three-quarters affected at least one strain. One of the clearest signals came from isosteviol, a compound derived from stevia, which significantly suppressed the growth of Roseburia intestinalis and Parabacteroides merdae when paired with the antidepressant duloxetine. Cambridge said those bacteria are important for gut health and for regulating blood sugar.

The findings, published in Molecular Systems Biology, add to a growing body of work examining how non-nutritive sweeteners interact with the microbiome. The researchers also reported that sweeteners can behave differently when mixed with other substances, such as medications, caffeine or food additives, which may change their effect on gut bacteria. That raises the possibility that the impact of a sweetener depends not just on the ingredient itself, but also on what it is consumed with.

The work is laboratory-based, so it does not prove the same effects will occur in people. Even so, Cambridge researchers said the results warrant more study, particularly because gut bacteria are increasingly understood to play a role in metabolism, digestion and immune function. For diabetes patients seeking sugar-free options, the study is a reminder that the safety profile of sweeteners may be more complicated than a simple calories-in, calories-out calculation.
