Researchers continue to explore metformin as a possible drug to slow certain aspects of ageing, but evidence supporting its effectiveness remains inconsistent. While it influences various ageing pathways, its role in extending healthspan or lifespan in humans is still unproven, with ongoing trials and mixed study results casting doubt on its geroprotective potential.
Researchers continue to test metformin as a possible drug for slowing some aspects of ageing, but the evidence remains uneven. As a widely used first-line treatment for type 2 diabetes and metabolic syndrome, it has a strong record for glucose control, yet its value as a so-called geroprotective medicine is far less certain. Reviews of the field generally agree that the drug influences several ageing-related pathways, but they also stress that the case for extending healthspan or lifespan in people is still not well established.
At the molecular level, metformin is thought to work through AMPK activation, which can influence downstream processes including mTOR signalling, mitochondrial biogenesis and autophagy. Other studies have also linked it to changes in the gut microbiome and to suppression of the senescence-associated secretory phenotype, a cluster of signals released by ageing cells that can drive inflammation. Even so, earlier reviews have repeatedly warned that these mechanisms do not yet add up to proof of meaningful anti-ageing effects in humans.
The best-known effort to settle the question is the Targeting Aging with Metformin trial, or TAME, a large placebo-controlled study designed to examine whether the drug can delay the onset of multiple age-related diseases in older adults without diabetes. The project has been repeatedly cited as a landmark test of the geroscience idea, but published results have not yet appeared. In a 2025 interview, Nir Barzilai said the work is now being handled by ARPA-H and could lead to two similar trials focused on GLP-1 drugs instead.
Existing human studies have offered mixed results. One major trial, MeMeMe, enrolled more than 1,400 people with metabolic syndrome and found that metformin at 1,700 mg a day helped prevent diabetes, including when combined with a Mediterranean diet. But the study did not show clear benefits for cancer, cardiovascular disease or mortality. More recent reviews and newer experimental work, including a 2024 report highlighting findings in male primates, suggest the drug may have some geroprotective potential, yet they also reinforce the same conclusion: metformin remains an intriguing candidate, not a proven anti-ageing therapy.
