Wider use of GLP-1 drugs reveals unexpected health benefits and risks beyond weight loss

As millions take newer dual-action obesity drugs, emerging research uncovers promising benefits like reduced inflammation and cardiovascular risks, alongside concerns over muscle loss and nutrient deficiencies, prompting calls for further targeted studies.

Millions of people are now taking GLP-1 receptor agonists and newer dual-action obesity drugs, and the wider use of these medicines is giving researchers a clearer picture of what they do beyond helping patients shed weight. A review by Steven Heymsfield of Pennington Biomedical Research Centre at Louisiana State University and Adam Gilden of the University of Colorado, reported by SciTechDaily, suggests that many of the unexpected effects so far have been favourable, including possible reductions in inflammation and lower risks of some cardiovascular and musculoskeletal conditions.

The evidence also points to possible effects well outside weight management. The authors said there is growing support for a link between these medicines and lower alcohol consumption, as well as a reduced risk of some substance-use disorders, although they stressed that more randomised clinical trials are needed before any firm conclusions can be drawn. At the same time, the drugs’ appetite-suppressing effect has raised practical concerns about nutrition, particularly whether patients may lose lean tissue, including muscle, while they lose fat.

That concern is not merely theoretical. Reviews of GLP-1 drugs have found that a meaningful share of weight loss can come from lean mass, and specialist guidance has warned that reduced food intake may also leave some patients short of key nutrients such as vitamin D, calcium, vitamin B12, magnesium and iron. Researchers say protein intake, resistance exercise and, where needed, targeted supplementation may help protect muscle and reduce the risk of deficiencies.

Heymsfield and Gilden argue that future research should focus on three questions: why inflammation appears to improve, how much lean mass is actually lost in controlled trials, and whether the drugs can genuinely curb alcohol-related harm. They said the overall balance of benefits and risks still looks strongly positive, particularly as the class continues to expand and more agents reach the market.

The same broader picture is emerging from other studies. A systematic review and meta-analysis published in Nature found that GLP-1 receptor agonists improve body composition, but also reduce absolute lean mass. Another review in Nature highlighted cardiovascular benefits, including lower body weight, blood sugar, blood pressure and inflammation, while a separate analysis in ScienceDirect estimated that lean mass loss accounts for roughly a quarter of total weight loss. Taken together, the findings suggest these medicines are doing more than trimming the number on the scale: they are reshaping how obesity, metabolism and long-term health are being treated.
