Blood sugar control is usually framed as a story about the pancreas, insulin, diet and exercise. But the kidneys are part of that picture too. Indian Express reported that they do more than clear waste from the blood: they help reabsorb glucose, make glucose during fasting and influence how the body manages excess sugar. Research indexed in PubMed and other medical reviews supports that view, showing that the kidneys are active regulators of glucose, not passive organs affected only after diabetes develops.
That matters because the link appears to run in both directions. Kanikka Malhotra, a consultant dietician and diabetes educator, told Indian Express that even small changes in kidney efficiency can affect how the body handles insulin and glucose before chronic kidney disease is diagnosed. Scientific reviews also note that the kidneys filter roughly 180 grams of glucose a day, reclaim most of it in the proximal tubule and help maintain blood sugar through a process called renal gluconeogenesis, especially during fasting.
The mechanism is complicated, but the basic idea is clear. Kidney cells use transport proteins known as SGLT2 and SGLT1 to pull glucose back into the bloodstream instead of letting it escape into urine, which is why a class of diabetes drugs targets those transporters directly. Medical papers also show that hormones and nervous system signals shape renal glucose handling, and that when kidney function starts to slip, insulin may stay in circulation longer and blood sugar control can worsen quietly over time.
The practical message is familiar but important: habits that support metabolic health also support the kidneys. Malhotra said staying active, limiting excess sodium and ultra-processed foods, drinking enough water and avoiding long periods of sitting can help both. For people with prediabetes, high blood pressure or a family history of kidney disease, regular kidney function and urine tests may pick up early changes long before symptoms appear.
