The effect is believed to come mainly from acetic acid, the key compound in vinegar, which may slow the digestion and absorption of carbohydrates. Even so, the evidence does not support vinegar as a substitute for diabetes medicine or a balanced diet.
A broad review indexed by PubMed found that vinegar consumption was associated with improvements in fasting blood glucose, post-meal glucose, insulin after meals, haemoglobin A1c, cholesterol, body weight and systolic blood pressure. A separate meta-analysis on PMC reported significant reductions in fasting glucose, post-prandial glucose, haemoglobin A1c and post-meal insulin, although the authors still called for more research before firm conclusions can be drawn.
Levels, a health site that has examined the issue in detail, says vinegar may work by slowing gastric emptying and increasing glucose uptake by cells, while Healthline and Medical News Today both note that the evidence remains limited and mixed. The practical advice across these sources is broadly the same: if people choose to use vinegar, it is better taken in moderation, often diluted in water or used in salad dressing before a meal, rather than swallowed neat.
Researchers also think vinegar may interfere with enzymes involved in breaking down carbohydrates in the gut, which could further blunt the post-meal spike. Some studies have suggested a small benefit when vinegar is added to food, including vegetables and carbohydrate-heavy dishes, and that it may help people feel fuller for longer. But the overall effect appears modest, and the available trials have often involved small groups and meals that do not reflect everyday eating patterns.
There are also cautions. Excessive vinegar may irritate the stomach, cause nausea or damage tooth enamel because of its acidity. For people with diabetes, the consensus in the material is clear: vinegar should be treated, at most, as a complement to established treatment, not as an alternative to medical advice, prescribed drugs or healthy eating.
