New research suggests coffee's effect on blood sugar is more complex than caffeine alone

Recent studies reveal that moderate coffee consumption may have minimal or even beneficial effects on blood sugar levels in healthy adults, but caution is advised for those with type 2 diabetes, as caffeine could impair insulin sensitivity in this group.

Coffee may give blood sugar a brief lift, but the effect is usually short-lived and not the same as long-term harm, according to Spanish-language reporting from El Diario and research published in Diabetes Care and related journals. The report cites metabolic specialist Mauricio González, who says caffeine can trigger adrenaline release and cause a temporary rise in glucose, while the broader evidence suggests that moderate coffee intake is not, by itself, a major driver of diabetes risk.

That distinction matters. A randomised controlled trial published in Diabetes Care followed healthy volunteers who drank four cups of caffeinated coffee a day for 24 weeks and found no meaningful change in insulin sensitivity or fasting glucose compared with placebo. The same line of research is echoed by observational work in Diabetologia, which found that coffee consumption was linked to lower fasting glucose, lower two-hour glucose and lower fasting insulin in both men and women.

Coffee without sugar, syrups or creamers appears to be the key qualifier. Another randomised, cross-over trial in Diabetes Care found that decaffeinated coffee altered glucose and insulin responses during a glucose tolerance test, which suggests that compounds in coffee beyond caffeine may also influence metabolism. In other words, the drink’s effects are more complicated than the common belief that caffeine simply pushes sugar up.

There is, however, an important caution for people with type 2 diabetes. Studies in The Journal of Nutrition and Endocrine Practice found that caffeine could worsen insulin sensitivity and post-meal blood sugar control in some patients with the condition, especially when taken before a glucose challenge. The takeaway is that coffee may be metabolically neutral or even favourable for many healthy adults, but it can behave differently in people already living with diabetes.
